How to Enroll

Enroll in the Utah Virtual Academy

Important Announcement Regarding Utah Virtual Academy Enrollment:

High school enrollment for the 2008-09 school year has closed. Students who have been offered admission from the waitlist for the 2008-09 school year have already been notified.

UTVA has reopened the waitlist for K-8 students. To obtain a place on the waitlist, families must (1) complete the online enrollment, (2) fax all required documents as listed step 2 below to 1-866-523-3160. Students will not obtain a place on the waitlist until both steps are completed. Seat offers will be extended to waitlist applicants as space becomes available. Siblings of currently enrolled students will be given priority on the waitlist.

The online open enrollment application for UTVA will be available between Monday, February 16 and Friday, February 20. If, at the end of the open enrollment period, the number of applicants exceeds the number of seats available for the school, a lottery will be held on Wednesday, February 25, 2009 to determine which applicants will be enrolled in the spots available. When a student is chosen via lottery for a space in the school, parents must complete the remaining enrollment forms by Thursday, April 30, 2009 to secure the offered spot in school.

If, at the end of the open enrollment deadline, there are still openings, UTVA will continue to accept applications on a first come first serve basis until all openings for new students are filled.

Apply Step 1. Complete Online Application

Please note that kindergarteners must be five years old by September 1, 2008, in order to enroll.

Forms Step 2. Provide Required Forms
After you have completed the online application, fax or mail the following required documents:
  • Copy of birth certificate
  • Copy of proof of residence
  • Immunization records
  • Utah enrollment acceptance form
  • Utah letter of withdrawal form
  • Proof of vision screening (if student is under seven years of age entering school for the first time) and/or in kindergarten
  • Charter school release form (if current school of attendance is a charter school)
  • Release of records form [link to PDF]
  • Official school report card/transcript (grades 10)

In order to expedite your student’s enrollment, please submit these additional required documents within 7-10 days of completing the online application.

Test
Step 3. Take placement tests (K-9 only)
If you are enrolling a student in grades K-9, administer the K¹² Math and Language Arts placement tests, which will help us to determine the appropriate courses for your student. Your student may start the placement tests (Step 3) while you are gathering the required forms (Step 2).

To administer the placement tests, log in to your Enrollment Dashboard and click “Begin” in the Tests column next to your student's name. You will receive detailed instructions there.

If your student is enrolling in grades 9-11 no placement tests are required. Instead, you will need to submit an official school transcript to help us determine the appropriate courses for your student.

Conference Step 4. Placement Conference
After you've completed Steps 1 and 3, a placement specialist will contact you to discuss your student's course level.
